You had the basics there but to get full marks you would need to include the following:
There is a title
There is a facility for scrolling for more data than the screen can fit There is a button to start over/clear data There is a button to save ingredient data/add a line of data There is a button to close the program Optimal use of screen Space for ingredient and recipe name long enough (at least 10 characters)
If I saw your form I wouldn't really know what I was looking at. Think about forms you have filled out online and how they are presented.
